Dictionary-like headers created automatically?

I’m creating a phonebook/dictionary-like directory where the first and last entries on a page are located at the top of that page. Does anybody know of any plug-ins, scripts, or methods I could use to help automate this process rather than manually type the first and last entry at the top of each page of the 200+ page directory?

A similar thread had been created once before, but did not find an adequate solution (thread818-855684)

Thanks Marcus, I appreciate it. I'll look into it. Does anybody know of any other solutions? Surely there must be more than one answer.
