Dial By Name Function Not Routing Properly

I have a Octel 250, Aria 3.0 with a Nortel 61C. The dial by name directory (type 30 mailbox) will allow a caller to enter/select the appropriate name - once it confirms the name and starts to transfer the call, the caller will hear music for a few seconds then the call disconnects. This is happening on both internal and external callers - this is the first time I've dealt with this feature so any guidance is greatly appreciated!

in the type 30 mailbox what do you have under the transfer supervision ? is it a yes or a no also what do you have in menu 6 1 for dialing sequence to transfer a call ??
