Dial-by-name duplicates

Hello, we are running CallManager ver. 2.3, sw version 12.9/CME 4.0 and are having a tough time with our dial-by-name configuration. When people hit our auto-attendant and are given the choice to dial by name, if we have multiple users with the same name, the system defaults on the first user. What do I need to do to allow callers to move forward to additional users?
 
The only way I know of to successfully do this, is to change the names. ie Fred Smith and Fred Smith become Fred A Smith and Fred B Smith. Then you have got to figure out how to let everyone know the difference.
You should be able to build a "greeting" so when either Fred Smith is chosen it goes to a choices menu saying press 1 for Fred Smith in accounting, press 2 for Fred Smith in Sales etc. This might end up being a pain to maintain though.
