Destination set contains hundreds of duplicates

I have a destination set that I just realized has literally hundreds of duplicates for several sites. Does anyone know what causes this and how to avoid it?

And, now that I'm sitting here deleting the dups one by one, does anyone know of a way to edit destination sets other than through the ISA UI?
