Definity login transfer

All,
We have a Definity site that has had a falling out with their former BP. They've asked us to assume service on their system but do not know any login credentials for either the Definity or Audix.
I've read several references in these forums about a "form" to have the user send to Avaya to create new logins and transfer their account to our GAI but can't find it on Avaya's web site.
Anyone know where I can find this form?

They will have to call a case into Avaya (billable) to get the passwords reset. I believe it it through the Avaya security croup. They will need to FAX a request on company letterhead into them.
