Definity G3Rv11 Autodial issues

We are having an issue with our autodial where 97% of the time the call will go through no issues. The other times a digit will call out and the call will fail. If we dial it "the old fashioned way" it works 100%. This issue seems to be very intermittent.

Here is the setup.

Analong DID(k2500), personal abbr dialing plan calling 877xxxxxxx**xxxxxxx#

Basically calling out to an 877 number and entering a pin code. Everytime this fails it drops 1 digit out of the autodialing.

Any suggestions or places to start diagnosing would be greatly appreciated. This happens on every line (11) that has this type of autodial. Could this be a tone detector card failing?
 
you probably need a ~p (pause) or a ~w (wait) between the number and the pin. Maybe even a ~p~m .

We do have pauses (*) in between the 877 number and the pin.

95% of the time it works perfectly fine. But the other times it is missing a digit. Do you mean putting pauses in between the passcode numbers themselves, cause I don't think there is enough space for that.
 
There is no ~m before the pin. What does the ~m do? Ive never used it in programming,

Also, with the * between the number and the pin it acts as pause. I did try ~p in place of the * and the only difference is the call fails to process instead of actually dialing and missing a digit.
