Defaulting MLM007 1

There used to be a form in the FAQ for defaulting a MLM007 back to factory specs. I'm been searching support.avaya.com, but I can't seem to locate it.

Anyone have it?

This is what I have in my docs for reset via serial port

Set the Hyperterminal to 1200 baud, 8 data bits, no parity, 1 stop bit, xon/off protocol
connect to the Merlin Mail
hit enter
choose shutdown option
after the shutdown press the Esc key once to get the 1D> prompt
type init and hit enter
reinitialisation starts (takes about 15 minutes)

if the menu doesn't appear type MM and hit enter
then start the voicemail
login passwords are reset to no password
