DECT 4 Licensing Issue - "No License Available" Despite Having Enough Licenses

IP Office Essential Edition R11.0.4.0.0(74), 3730 cordless, Provisioned install. The site has 12 licenses for the phones, and only had 11 phones subscribed, so a new phone was added and subscribed. The phone worked normally for a few months and then today started showing a "No License Available" message despite nothing changing the in the config. We tried reserving a license, not reserving a license, turning off other phones, and deleting and recreating the extension and user and resubscribing. All attempts failed, as the phone will subscribe but then switch to that message. One thing we didn't try was deleting another phone instead of just shutting it off, but that's about it. We've replaced 3730s at this site before, so my first thought was one of the "lost" phones that was replaced was somewhere in the building and powered up, but checking the Users on the Master base station doesn't show any extra phones subscribed.

Any ideas on what's up? Licenses aren't expensive so we'll likely just add another one if the juice isn't worth the squeeze on troubleshooting, as long as adding a license solves the issue.
 
Usually I don't use provisioned install at all 'cause I don't have full control of the system and IP dect config/behaviour. Anyway I would check the master IPBS if the subscription is enabled or not (should be NO for security purposes) to check if any other device has been subscribed in the IPBS.
Then have a look at the System Status to verify how many licenses are used and/or free.
