Dead S/L ports

We have a customer with a full 3 cabinet Legend R6.0 with a full mix of single line and MLX phones. Occasionally, about twice a month, one or two single line ports appear dead. There is no dial tone, no audio and no ability to dial, yet if you call the phone it does ring. If you reseat the module the phone returns to normal working condition. We have replaced components, yet the problem still exists. Unit loads on the power supply are adequate. Any insights?

if the card sees error in the wiring or the telephone set it will disable the port. change the port and plug a new analog phone into it and see if it goes dead.

There is a known problem with the older 016 cards.
You may want to try a Firmware upgrade is it is am 016-U

The following escalation codes are fixed with the upgraded 617F34 module.
