Did any one come accross this before for VNS.

Not sure what VNS is but DCH 23 is a minor loop issue and DIAG 289 is from the carrier. Could be a media card related issue on your side though ... similar to MC32S (TDM to IP conversion) - we had quite a few issues with those card resources going down or not coming back up after work performed.
Good luck figuring it out.

VNS is Virtual Networkign Services and is an IP dchannel I suppose. I think the issue was hardware/path related.
