I will make this 1.5 day fiasco short and sweet.

Issue:
IPO staged in lab with 2 DBM32 button were installed on 2 1616 sets (with proper POE brick)
After installing at site one of the DBM32 failed to flash or go live on power up.
Amazingly, the DBM32 worked when you plugged directly into IPO...out at station jack..dead.

Bottom Line:
The Ethernet cable packaged with the POE brick was defective.
My associate tested the cable with his continuity only cable tester and it PASSED.
I tested with my slightly more advanced cable tester that does a minor data test and the cable FAILED !
Why did it worked right at IPO but not at the location...who knows..
Note: As you know, that patch cable goes from the Power brick to the phone and the Phone WORKED but not the DBM32.

Changing the cable to one of those red ones that came with IPO...DBM32 instantly worked.
What screwed me over in testing was that we started testing and mixing all the variables (phone, brick, cables)prior to knowing one of the patch cables was bad.

On a side note, last fall we installed 120 IP phones with patch cables supplied by Avaya and...believe it or not...60% of the phones would not come up. Tested the cables and fouins the exact same problem as above. These were NOT the same cables as in the problem above...different color & texture. We had to scrounge up new cables and retunr the Avaya cables.

Luckily they do not provide those patch leads anymore as they are the cheapest you can get and are bad.
Get some connectors so you can make your own (dbm) cables.

BAZINGA!

I'm not insane, my mother had me tested!

I can't believe this[peace]

The same issue happened to me on a DBM32 module
I had to change the patch cord that came with he module.

When I pushed it in hard in the back off the phone it would flicker on.
Crimped the ends again and did not work.

Replaced the patch cord and it worked right away.[ponder]

You need a BM32 for a 1616, the DBM32 works on a 1416.
