Database / Merge Question

I have a (probably) a simple question....I just don't want to reinvent the wheel that someone else, I'm sure knows!

I know someone who is in the medical field. The way they do chart notes is a bit cumbersome...

We will have a database of symptoms, diagnoses, recommendations etc...

Then we will want to merge that with a skeleton document, drop in that patients diagnoses and print it out...
then take same database info and drop into a patient chart...print it out. I can do this with a word doc template...but would be neat to have a page that we could select from drop down lists and have it pop into a printable doc. I could also use ultradev, but not sure how to get that to print out in a decent manner...anyway, just looking for thoughts, ideas and input!

If you want to do this web-based, that will not be to difficult to do in ASP.
If you want to do this as an application, you should be able to do it relatively easily with VB (provided you know some programming)
If you would like to continue using WORD templates than you should look into a product called ActiveDocs. ( ). Using active docs you can create templates that will have active fields and database access to create templates. In this fashion you can create a "form" template which would have certain fields tied to the database so that you could select from the data in storage. This way you could create a standard template and continue to save all of your files on word format if you wished.
