the most common cause is the file path provided by the database for this file is not accessible to the Livelink server - this may mean that the EFS is wrongly listed in Livelink or just that that actual file is no longer in the EFS so Livelink can't retrieve it.
If it is most/all files it is likely to be the former, if just that one then it is likely to be the latter.
The Livelink Thread logs will show you the path to the file Livelink is looking for, so get that and then log onto the server as the user that runs the Livelink services and see if you can get to that file.
In my case, I have checked that the file is no longer exist, but now I'm wondering why the file is not there anymore although it still can be searched via Livelink?
Livelink Search looks at what is in the index, supported by the database. I assume that as you found the file that the Livelink database "believes" that the file is still there when the actual physical file is not.
I would check the audit trail to see if the item has been deleted and not cleaned out and also check the audit on your EFS to see if you can identify the cause.

also if you find the 'manage database'' and 'verify database' ' links there are verification steps that give you a report of the anomalies
Well, if I called the wrong number, why did you answer the phone?
