cut/paste jpg shows file name instead of image

Pasting a jpg image into a word file shows the file name (ooo.jpg) rather than the image. this is ms word '03. The behavior is new. I have checked around and don't find a solution. Clicking on the file name opens the image in MS viewer.
I am sure there is a simple solution. Note that the show place holder setting is unchecked.
