Customize Acrobat form using VB

I am looking to be able to create from a generic template a customized Acrobat documents.
Using a program VB or other, I would like to be able to use the template and change the name/address to create the customized document.
Is-it possible to do this? And is there a link that could show me how to do this.
Thanks for any clue.
