Custom Voting Forms in Outlook

OK, here's what I'm trying to do.
I am planning to set up a web page on our intranet. It will have a list of groups on it which will all be linked to distribution lists using the mailto: tag. What I would like to do is for the mailto: tag to launch a form template in Outlook instead of just an ordinary e-mail. The idea is to enable the receiving party to simply reply by clicking a "Yes" or "No" button so as to save each of them from typing out vast amounts of text each time they can't help with the problem being reported. I want it to work in much the same way as it does in Outlook when someone sends you an appointment request. You can either accept or refuse.

you can send an HTML e-mail with a form where the action will be the script that you will use to collect results.
