Custom Outlook Forms

I have created my own help Desk form for our company, that allows users to to request help from IST, and automatically posts reponses and resolutions to an Access Database, However, it doesn't work for some users, when they post the request, it looks like the send operation didn't resolve the name. the user gets an &quot;Operation Failed&quot; Message from outlook.<br>

Any Ideas???
