To create a custom letterhead you need to modify the design of your mail db (traditionally not a good idea).
In practice, all you need to do is copy an existing letterhead subform and modify it, give it a new name and a correct serial number (increment from highest existing).
That is easy, really, but the restrictions are less palatable. Normally (unless R6 changed something about that - which I doubt), the letterhead is not transferred per se, only the number reference. Therefor, it will only be seen by those who have the same design as you - which will be nobody if you do not make it a corporate standard.
Of course, browser email clients will not see it since they cannot possibly know what to do with a number for which they have no reference.
You may think that someone can send you a letterhead, but that is simply because your correspondant uses Notes and he transmits his letterhead number which your Notes can use.
If you look carefully, you will make the difference between the default letterhead and the others. People who do personnalize their letterheads beyond the standard set will send you mail showing up with the default letterhead (since Notes has a number ref it cannot handle, it shows the default).
So, you can personalize as much as you like (if you have a Designer client), but in all probability you'll be the only one benefiting from it.
