Custom Form Fails

I created a custom for based on IPM.Post to store client data. When I try to save or post the form a exclamation box pops up with the message "11/05/2001 8:00:00" and nothing else. The Item_Write event never fires and the item is not saved. I can close the item but when prompt ed to save changes and clicking YES I get the same exclaimation.

There is no code behind the form (no vbScript no VBA, no add-ins). It is just a modified POST form with custom fields.

Any help would be greatly appreciated.
