css vertical menu does not display w/ a form

i am having a strange problem with a css pop-out menu. it will not display of there is a form on the page. has anyone had this trouble before? thanks -

the pop-out portion of the menu does not display at all. i have the form at the bottom of the page, so no elements are near the menu. thanks -

Also, what browser(s) does it fail / work in?
