CrossTalk/Cross-Connect issue thoughts?

Hey there. When I pull a Call by Call Report it looks like the call is coming from our Dialer system to land on the DN of an agent so they are reporting cross-connect issues. I am pretty sure this is not the PBX (CS1000E) causing the issue. I have traced it to route 21 which holds the dialer PRI trunks that are loops 115-119. I have been unable to trace a call because they report it too late.
The dialer is integrated via CTI so agents can do outbound and inbound.
The dialer was rebooted and the problem seemed to clear for a few days.

Thoughts? It has been working fine for months and there have been no changes to trunks or anything of that nature. Maybe a carrier issue on the dialer trunks?
