Crossed calls on VOIP

We have an intermittent issue where calls get crossed. Agent 1 receives call and about a minute later a second agent gets the call. It appears to the second agent as a new call with the whisper. Both agents and customer can hear each other. The second agent just hangs up without effecting the call. All crossed calls were results of internal transfers.
 
OK so need your hardware set up , cm version , what type of trunks ,.....what have you done to trouble shoot this ...can you replicate the issue in real time by throwing calls into the switch ?

Are you using any other call handling methods other that a hard phone?

As much information as possible please , with a idea of logical troubleshooting you have tried already.

Also, if you have Medpro's and CLAN's, whats the firmware level of those cards.
