Creating Forms in Outlook XP

I am new to creating forms in Outlook XP (Exchange 2000). First off, does anyone have any good resources learning how to create customs forms in Outlook XP? What I am trying to to is create a form that basically has attributes of a normal contact form, but can also have taks associated to it. I want to use it for customer relations. Basically, I want to be able have my clients enter contract information, than if they call the contact, they can add information about the call. If the client says to call back in six weeks, they can add that, but unlike a normal flag, I want it to send a noticfication when it is due. I would laso like to try to have this info viewable in a calendar. All this info will be in public folders. Does this make sense, has anyone created anything like this, or a form for Customer relationships? Thanks for anything!!!!
