Creating a Table of Contents when converting an HTML doc into a PDF

When converting an HTML file into a PDF, I would like Acrobat to automatically create a table of contents. Is it possible to do this and if so, is there a script to do this?

You would do better to do this in MS Word which has a simple facility to create TOC based on formats such as H1...etc.
