Creating a Report based of form query

I've never tried this as I don't personally use Filters, but have a try anyway.
In the On Open Event of your Report:
Me.Filter = Forms!YourFormName.Filter
Me.FilterOn = True
In the On Close Event of your Report:
Me.FilterOn = False
