Creating a Questionaire form in Adobe Acrobbat Pro. 7

I am using a preconfigured template in Acrobat which works great but only contains 10 questions. I would like to add more questions to the template. I created a second page and copied and pasted the orginal questions to this page. But when I submit the form using the email submit button the .xml text reveals this: How do I edit the form so the new questions added will have a different reference # so when exported to Excel a separate field is created for each question or comment.
