Creating a form that omits some data

I have a data set some 300 rows and 15 columns; the first column contains the names of 3 people. (100 rows each) the other 14 columns actions the people must perform.

I have merged this into a word document, however, and as expected I get a new sheet/report for every record appearing on the data set.

I would like to see a form the data set a persons name (displayed once at the top of the page) and all their tasks displayed underneath.

I would be grateful for any help in achieving this

Please type in a few records as they currently exist and how you would like to see them in Word. Is there a reason you don't use an Access report?
