create simple chart

Hello,
I'd like to create a simple chart with x-axis: scott.emp.ename
and y-axis: scott.emp.sal
This chart should be created with Forms 10g.
I took a look at the forms10gdemos but did not succeed.

I think the my main problem is the record_group.

Please help
