To apply it, press Ctrl-J (whilst in your project) and on the bottom right you can attach an icon. PLEASE NOT foxpro is very poor in its icon handling a full colour icon will attach but doesn't work, use a 16 colour icon only (this sux but otherwise it doesn't work right) Also add the icon to your project (under other)
Your ICO file should contain at least two sizes of icon: a 16x16 icon for use in "small icon" folder windows and on the taskbar; and a 32x32 icon for desktop shortcuts. Depending on your needs, you might also want to include a 48x48 icon.
If you can't find a ready-made ICO file that you like, you should get hold of a good icon editor. The one I use is called MicroAngelo. I can't remember where I got it from, but you could try a web search. It is not free, but there is a trial version. With luck, you will be able to create your icon before the trial period runs out.

Only to add to joemcs's comment about the icon needing to be 16 colors only, and although Microsoft itself states it cannot be done, I recently came accross an article that explained how to get a 256 color icon in a VFP application using a program called "resource hacker". But this was not the point of your question. Mike Gagnon

Another alternative to creating your own icon is to extract an icon from your own system and save it in both 32 x 32 and 16 x 16 format which will enable you create an icon suitable for VFP.
My reccomendation for an excellant icon editor is "Icon Forge" from Cursorarts.
It easily creates the multiple size icon file referenced above, and is free to use for 20 days. The 20 days is calculated as *actual* days you use the program, instead of 20 days from installation.
you can also make custom icons by editing a BMP pic using MS Paint (comes with Win 98 SE). this pic is 32 x 32 in size (actually any size will do as long as the dimension is square)and will be saved using only 16-bit color. then after saving, change the file's extension name from BMP to ICO and viola! Windows will do the conversion for you. try it. it works!
