Create a Scrap Report! HELP!

Hello!
I need to create a scrap report that includes the following:

DATE
OrderNumber
ItemNumber
WorkCenter(WC)
EmployeeNumber
FirstName
LastName
QTY-Completed
QTY-Scrapped


I need to be able to sort this table by a given day. EX. (7/10/2012)

I think I have the right tables, but I believe my linking is wrong?
I need this report quick!
