I am trying to think on what i should include on a cover page for a project plan,

so far i have the name of the project (probably wouldnt live it down if i forgot it)

I got the aim of the project,

The deliverables

and the controls

any suggestions on what else to include ?

I would look at it like an Executive Summary.

Cover page? Name of the project, your name, date, revision number of the document.

The document itself? Search the web for Statement of Work or Project Charter. You'll find that you should be including a _lot_ more stuff than you have mentioned.
