I'm guessing it's fine to use the icons supplied with Visual Basic, but what about ones in EXEs, DLLs, etc.?
Would one need permission to extract and use these? Does it depend on the software (shareware, demo, freeware, etc.)?
 
Each EXE and DLL should have legal verbiage which discribes how it can be used. If it does not say that the icons are freely usable, I would not assume they are.

Also, not all of the code bundled with VB is distributable by you, either. Even in VB 6, there are some libraries where Microsoft has license to distribute the code, but where their license does not transfer to you. There are a lot fewer problems with VB 6 than with VB 5, but I'd start reading the fine print before I distribute. ______________________________________________________________________
