constant qos errors 61c rls 6.0

This pbx has been installed for years with no issues but almost everyday for the past few months we have been getting qos errors. We have remote users who work from home, they are a mix of reach line card sets and I2004 sets and they are the ones noticing calls dropping and voice quality issues. We have one MC32 card for our VGW's which I notice sometimes coming up qith qos errors if a IP set dials a digital set I'll see the MC32 card involved with some latency or jitters. Most of the time I see qos errors pop up between 2 remote ip users. Where should I start looking for these errors, or is it our carrier coming into the building causing the trouble? Below is a few errors that just popped up, sometimes I see packet loss at 99%!

Did a recent network switch replacement or router change happen? Those are warnings that you are approaching latency thresholds and seeing some jitter.
 
Possibly, parts are always changing around here. Should I be checking on the Elan side or the Tlan side for this?
