Console unable to blind transfer calls

Since upgrading to 4.2 SP2 our client says they can no longer blind transfer from the Console. They will only go if far end (or VM) answers.
Any Ideas?

Make sure your COS's haven't defaulted.

I've seen this happen when relicensing a system causes the Mitai option to be turned off and then any COS with HCI enabled will fail to restore.
