Connecting to a remote SQL Server database with Crystal Reports

How do you connect to a remote SQL Server database with Crystal Reports?

If you can connect to the database server from the machine where Crystal resides, Crystal can connect to it.

There may be firewall and other considerations.

Define *remote*.
