Config password phone for users gone after random times

Hi,

Need to ask another help if you dont mind,
One of the avaya ip office on different sites have this kind of issue, suddenly the configuration for password just gone at random times (been twice already) and only the config for phone password, are there any config / logs / policy i can check regarding this beside check the audit trail which at near times of issue no one change anything in configuration ?

Appreciate your advice / thoughts regarding this.

11 - & yes other site use 10 version never face issue like that. Is it due the version 11 ?

Lock the system down, change the passwords, give the Admin access to only Administrators, any other people logging in give them Manager Access. Change the Security Password. Login only using secure communications.
Is the system IPO Office PBX or Sever Edition?
Passwords would be stored in NVRAM would be very strange for this to be faulty and only the Config password changed. Suspect foul play and increase the security around the system.

Yes. There is a bug in release 11.04. Which it deletes user rights or passwordS from the user.
Its fixed in R.11.04.2 sp2. Read the release note for full fixes list.
