Conference Outside Line Not Working

HELP! I know this is an easy fix and probably just overlooking the obvious...have an S8300, when conference button is used to conference two "off site" calls, it does not work. the first call goes on hold as it should, second call connects, however, when conference is pressed the second time to bring all parties together, the magic does not happen and the button the second call is on flashes momentarily. checked COS and it looks right unless i am missing something...which i'm sure i am! Call Fwd-Off Net is restricted but didn't think that affected the conference function. i see extended forwarding and call forwarding advanced are both off, but i tried turning them on and that didn't do anything...

saw some other threads similar to this one, had someone onsite attempt the transfer and did a list trace when she did - here's what i got:

is that a LEC issue or perhaps something in the trunk group?
