Conference Center - Simultaneous Calls

I have had recurring problems with the conference center not allowing simultaneous access. When you dial the conference center number, you are prompted for the user id, enter code - one gets in, the other is told the conference center is not available. When you re-dial, you get right in (when uncontested).

IP Office version 5.0.18; VM 5.0.25

Here is what we have done to resolve to date:
- move from voice mail server to web server
- move from web server to stand alone win 2003 server
- Only one copy of conference center is resident in network
- reserved vm ports for the autoattend (4)
- Established a separate inbound group pointing to a short code, also have DID pointed to Conference Center
- Tested to see if the conf chip in the unit was bad - added 4 people to a conference bridge - OK
- updated from 5.0.18; VM pro version 5.0.25

Do not see it as capacity as it seems to fail when the VM PRO talks to conference center program. We have run the vm debug which supported this conclusion. This is why we moved from the web server to win 2003 stand alone. All was well for about a week - now we are back to square one.

I am not sure where to go from here. The customer is quite frustrated (and us).
