Conference calls dropping

Has anyone seen this? Here's the information I am getting from the onsite tehcnician in Pennsylvania.

An executive in San Francisco is creating a conference call. During the call, one of the Directors in Pennsylvania gets dropped off of the conference call a few minutes into the call. No one else is being dropped from any other location. This has happened to her several times over a period of 6 months or so. 1 to 1 conversations in this center have never been affected, only conference calls. Where should this technician look first? Thanks.
 
When SF calls PA to do a conf call, who originates the conf call? Is the attendant setting up the conf call in SF? Does the call terminate on a DID in PA? Have you been able to duplicate? Are other users being effected when conferencing? Some executives are above establishing conf calls, thats why I ask.

In response to your questions, we conducted a test between Pennsylvania, Oklahoma City, and myself. We left our conference open for an hour and the call never dropped. If I remember correctly, we originated that test call from PA. So in short, no, we were not able to duplicate the issue.

I'm not sure if the Executive or the Assistant is setting up the conference call, but it only seems to drop the user in Pennsylvania. I thought it might be an issue of trying to connect more than 6 people, but she is always the one to get dropped. I guess it could be due to the fact that the conference call may be initiated the exact same way each time. (i.e. SF, then OKC, then Vegas, then Rocklin, then Memphis, then PA.)

It is a DID in PA.

There have been a couple of other instances where other users in PA have been dropped. I just don't have all of the details as to whom they were all connected to and who originated the conference call.

I may need to get the onsite tech to get some more information for us.
