Conference call _ TSE 4.1 sp6

To explain better, I can not gather in the conference two IP extensions, the first one remain in on hold status even I press conference button. I tried also clearing the second call and going back to the first call but without success. The first call remains in on hold status.
If I call first another digital entension (tried with digital extension in the same mgw as the initiator) and then two IP extension conference goes correctly.
Someone else have noticed that?
