Conference Button on Phone

I have a 500v1 running 4.2.11 and VM Pro with a PRI.
While on a call we hit the conference button and add someone into the call, once all parties are on the call nobody can hear anyone.

I can hear the first person and hear the second person but once I conference everyone together everyone is silent, and I can see that the conference is setup in system status.

Dodgy conference chip I assume, or you could give it an upgrade to R5 and see if it sorts itself out :)

Makes sense since they think that it use to work but then it stopped. Since R5 is a unlicensed upgrade I can start with that.
