Conference Bridge intermittent issue

We have been rolling out the use of our conference bridges and have run into an issue:

At least two outside callers who are calling in from their home phones have had a lot of difficulties connecting into active bridge calls.

1 user connects and received the prompts but their digits are not accepted.
Another user connects but does not receive the prompts.

we have tried duplicating the issue with similar carriers (AT&T and Comcast) without luck and my boss wants to know what our next steps are in trying.

Any further suggestions?
 
Is it the same two callers that have issues ? Or just when any two callers try and connect they have issues ?
 
Also, what release IPO? What type of chasis (406, 403, 406V2, 412, 500, 500V2)? And what type of lines are you using for the IPO (PRI, SIP, etc)? This will help others to assist you.
 
If it's always the same two callers and all others are fine it's unlikely to be the programming or systems lines, it would indicate an issue with those callers lines/service specifically :)

As a Work Around can you have the 2 callers call someone who has an iPhone or cell phone and then have that person with the cell call the conference bridge and merge their call?
This might get you through until a solution can be found.

Or maybe they can call someone's extension who will be on the call at the place of business and then you can setup their handset to have a programmed button to transfer to the confernce bridge.
