Conference and DTMF failure

We have a Meridian 61C and M3903 phones. At times the 'Conf' key does not work. It happens randomly with any phone set. It is also observed that very frequently when we interact with an IVR and have to press buttons for DTMF, we don't get the desired effect. Sometimes the tones are clearly heard, at other times they are even not audible. Can anyone help to pinpoint the problem?

perry is right it could be a supervision problem,

And on your FSUM print there is a problem in that you are behind in the firmware on the 3903 sets

Your current software 25.30 expects that your 3903's run the firmware thats on your disk-looks like you may have upgraded system to 25.30 and never upgraded your firmware on the phones -in ld 22 , REQ PRT,TYPE PWSV, AND LOOK FOR 3903, IT SHOULD SAY 52.
