Companion Phone Rings Constantly

We have a model 3060 companion phone that rings constantly; when you open it to answer you get dialtone. The switch is an Option 11C Release 22.16.

Any ideas what could be causing this? The other 10 companion phones work fine.

could it be a faulty phone, try swapping it with a good one,

I fixed it. I outted the phone and then added it. Problem went away.
