Any ideas as to why I'm getting "Communications error: disconnected (carrier lost") when I dial into a particular switch using the telephone number, but the IP works fine when I'm on the network?
I've used the same connection properties, laptop and telephone line to connect to a switch in a differnt location, with no trouble. I've already tested the line by calling it and I got modem noise, and I tried reseating the analog line to the modem.

It could be (and often is) noise on that line. Of course that is why you have no problems accessing it via IP. You could try baccking the speed down on your connection to the analog line as this will make the connection less likely to drop even if it is noisy.
