Combo Card Problems with new PRI install

I have a customer with an MICS using DID trunks and CO lines currently. We are in the process of adding a PRI. Yesterday I powered down the system, added the T1 card and swapped out the 6 port fiber card with the combo card. When I powered the system back up everything came up with the exception of the sets on module 4 (which is connected via fiber to the combo card). All of the sets on module 4 just blinked and blinked into oblivion. When I powered it back down and removed the combo card and put the original 6 port fiber card back in and powered it all the phones came up, including the module 4 sets.
What am I missing here? This should have been a simple card swap up to this point.

Any help would be greatly appreciated.

maybe a bad port on the combo card? Did you try an empty port?
 
if everithing was properly connected (wich i do not doubt) you have a defective combo card
 
Well, back on site working on this again. I decided to try to "add" the combo card instead of replace the existing non-combo card. Plugged it into the open expansion slot, with no fiber plugged into it and powered the system up. Some phones flashing (those not on KSU) and others "In Maintenance"
I unplugged her again, removed the combo card and she is back up and running with no problems.
Should that have worked? I am not even sure if I can add the Combo card as a second card without the 1st card being full.
Does this still sound like a bad combo card?
That is what I am leaning toward, although I have never come across it before.

you cannot install combo in slot 1 - without having XC software ...thats a different problem

But it cud be a bad combo card
