CO calls forward to DISA

Hi there!

I have an NSX1000 v006.00015, I'd like to redirect the customers on call to a DISA message floating extension number, where the caller can choose a dial number to redirect another DISA group.
My problem is that when we are in call with the customer, I cannot forward them to a DISA floating number.
My goal is a customer satisfaction survey, it should looks like this: [customer mobile] > our call center guy speaks with the customer > end of the call he redirects the customer to a DISA message "please choose 1-5 ratings on dial numbers" > after dialing the right survey number DISA message tells "thank you for the survey" > end of call.

Can you please give me some advice?
