CM "glitch" or misfingering?

Working with Definity/CM/Aura for over 18 years and never heard of anything like this before-

Sequence as described by secretary:

User is on a call with a Client 'A'

Client 'B' calls User. Call goes unanswered and covers to secretary.

Client 'B' asks for User or User's Assistant.

Secretary transfers Client 'B' to Assistant who does not answer.

After several rings secretary places Client 'B' on "Hold".

User is surprised when they discover Client 'B' now connected into same call with Client 'A'.

Secretary reports when placing Client 'B' on Hold their finger might have trailed down and accidentally pressed the Conference button directly below the Hold button. So far unable to duplicate. Curious about possibility of conferencing 'A' & 'B' by secretary when 'A' not active on secretary's station.

Secretary has Busy Indicator for User.
No bridged appearances.
Running CM 6.2
All sets are 6416D+
 
list trace the scenario whilst recreating the exact call flow with you dialling in and see what happens.
