That really depends on how your server is configured. If you use an email server that will scan incoming messages for virii, then you could set it up to either tag messages with malware attachments or delete them immediately from the queue.

Whoops, I didn't notice that this was the qmail forum when I posted that. I gotta lay off the cough syrup.
So your question is about qmail and clamav?
I've seen some good instructions over at that will allow qmail-scanner to delete messages based on spamassassin, not sure about clamav though.
