Cisco WS-C4006, one blade status light change to orange!

Hi,
One Cisco WS-C4006 Switch. Recently we bought one WS-4148-RJ45 blade and put into slot 3. the Status of this modeule shows orange, if the computer connect to network through this blade, and there is no problem.

1. We tried this new blade in another WS-C4006 Switch, the status light become green after inistial, so this new blade should be normal
2. We swap two blade in this switch, and only whichever blade you put into slot 3, status light change to orange.
3. We get RMA of the Chassis, and replace chassis, and we got same thing, no change.

Check to see if you have identical power circuits on both of the 4000.

Also check the environmentals on that 4000. I beleive the command is "sh env power

I have compared the configuraion of different blade in this switch. And there is no big different, I can not see anything special for Slot 3 and cause the status light change to orange.
Like what I said, I tried to put anohter blade into slot 3, and we got same result.
It is very strange.
 
It has been fixed, I got RMA from Cisco to replace Supervisor, and after that, the status light change to Green
